The root cause of the issues:
• The first major factor is the lack of proper leadership and management guidance at the eastern dairy company.
• There is no clear policy and set procedure that reminds the employees of the code of conduct and work procedures.
• The company has a bad inventory management and a non-existent quality control department. There is no defined maintenance procedure and schedule for the equipments.

• Set up a quality control department that will monitor all the incoming and outgoing inventory as well as ensure that there are checks and balances at every process step. There should be quality checks at every step and any kind of deviation should be reported and should disposition based on set procedures. The company should also strive to get the quality control certification that will set it to the global FDA standards.
