For this reason, I will introduce and suggest an alternative way to grant his wish without compromising the truthfulness of my relationship with Mrs. Newcomb. If Mr. Newcomb still prefers to hide his concerns from his wife, I have to maintain the veracity and integrity of my current patient relationship. To guide me in this ethical decision making, I have to determine and review if there are mandates that will contradict with my scope of practice or any hospital policies that will be violated in case I will grant Mr. Newcomb’s request. I have to discuss this matter with Mr. Newcomb’s attending physician and my nurse manager to endorse the situation and follow the chain of command and to collaborate with other healthcare professionals. If granting the request will hold me professionally accountable, I have to decline Mr. Newcomb’s request, and explain factually the reason behind it. However, in case there are no policies or mandates that will be violated and the nurse manager and attending physician authorized me to abide my personal wisdom and do the thing which I believe will uphold the best interest of Mr. Newcomb, I have to respect Mr. Newcomb’s autonomy and uphold the confidentiality of our conversation. I believe that the core principle of hospice care is to allow the patient dies peacefully as much as possible. If allowing him to see his mistress for the last time gives him peace, then I would grant Mr. Newcomb’s wish.
